Capital Request Application Procedures

Please include the following in your capital grant proposal to VNA:

1. Pages 1-2 of the Chicago Grant Application Form completed.

2. A narrative that includes:

  1. the history and mission of your organization, the population served and the services provided;
  2. a description of the capital project for which you seek funding, the need for this project and how, when completed, it will satisfy the need; and
  3. the timeline and plan for the successful completion of the capital project. 

3. Budgets, including a current year operating, capital campaign, and project/proposal budget

4. A Board of Directors list, and a listing of the capital campaign leadership (if available).

5. The most recent audited financial statements and annual report, a list of funding sources for both the agency and the capital campaign, and the agency’s most recent IRS Determination Letter stating its status as a 501(c)(3) organization with a 509(a)(1) or 509(a)(2) designation.
