Reporting Guidelines

A six month check-in and twelve month report are required for all VNA Foundation grants unless otherwise requested in your grant award letter or by your Program Officer.

Note: Refer to your grant award letter for your report deadlines and grant reference number.

Please read the following general information before preparing your report.

• Answers need not be lengthy and may be written in a narrative form or, if appropriate, using bullet points.
• One page for the 6 month check-in (or a 15 minute phone call if requested by your Program Officer) or three pages for the twelve month report are often adequate.
• Note that there are three 12 month report templates. Contact your Program Officer if you are not sure which template you should use for your report.
• Feel free to contact us if you have any questions or concerns.
• Reports should be mailed to the VNA Foundation at 20 N. Wacker Dr., Suite 3118, Chicago, IL 60606.
• Reports may also be sent via email to :
• Call your program officer at 312-214-1521 if your report must be submitted late.
• Do not FAX, Fed Ex, UPS or Priority Mail late reports unless requested.

Six Month Email Check-In:

1. Please describe the overall progress of your grant project and/or funded activities so far. Are you making progress toward your outcome measures? Are you experiencing any challenges?
2. Do you anticipate any difficulties in completing your project and/or outcome measures in the timeframe outlined in your proposal? Do you feel it is necessary to revise any of your outcome measures?
3. Are grant funds being expended as planned?

Choose one of the following templates for your report. Contact your Program Officer if you are not sure which template to use. 

Twelve Month Report #1 (for use by grantees who are not seeking renewal or subsequent year funding) 

Twelve Month Report #2 (for use by agencies seeking renewal funding, as invited by VNA) 

Twelve Month Report #3 (for use by multiyear grant award recipients seeking subsequent year funding)
