Reporting Guidelines

A six month check-in and twelve month report (also called follow-ups) are required for all VNA Foundation grants unless otherwise requested in your grant award letter or by your Program Officer.

Note: Refer to your grant award letter for your report deadlines and grant reference number. Additionally, you will receive an automatic reminder and link to report templates (follow-up templates) from our online application system.
