Reporting Guidelines
A six month check-in and twelve month report are required for all VNA Foundation grants unless otherwise requested in your grant award letter or by your Program Officer. Please submit your report through the online grants management system by logging on to your account and finding the appropriate report/update form. If you did not apply through the system, please contact your Program Officer for reporting information.
If you would like information about renewing your grant with VNA, please see Renewal Funding, then contact your Program Officer.
