| Purpose and Goals
The Super Star in Community Nursing Award will:
- acknowledge the service that Public Health/Community Health nurses provide both to individuals and to communities;
- recognize the value of nursing in the health delivery system;
- call attention to the national and Chicago-area nursing shortage.
The Award
The Award winner will receive $25,000 from the VNA Foundation, and up to five finalists will receive $1,000-$5,000 each.
Who Should YOU Nominate?
The winner will be a currently employed, practicing nurse delivering direct community, public, or home health nursing services in a “front lines” setting who has evidenced outstanding performance in one or more of the following categories:
- Clinical care: Excellence in direct nursing care
- Client advocacy: Exceeding the call of duty in advocating for clients
- Innovation/creativity: Innovative and creative contributions that improve the health of the population served
Many nurses take for granted the challenges they overcome every day. We believe that these remarkable stories should be shared--help us share them through nominations to the Super Star Nurse Award.
Eligibility

Nominees must have a valid RN license and work in Chicago or the Illinois collar counties (Cook, DuPage, Kane, Lake, McHenry and Will), and be an active, current practitioner.

Past nominees are eligible; past winners are not.

Nominees must be available the morning of June 10, 2008 if invited to interview with the award panel, and the award recipient and finalists must be able to attend VNA’s annual award luncheon to be held September 4, 2008.

Information about the winner and finalists may be disclosed by VNA via its website and other means, and the award winner must agree to participate in public relations efforts related to the award.
Process
Judging

Nominations will be judged by the VNA Foundation and a panel of public/community health experts.

Some nominees will be invited to be interviewed by the panel the morning of Tuesday June 10, 2008.
Other Information

The $25,000 award, and all other cash awards, are unrestricted and may be used for any purpose.

All taxes are the responsibility of the winner(s).

Nominators may nominate more than one nurse.

No self-nominations will be accepted, nor will nominations of a nominator’s direct supervisor.

If an individual is nominated by more than one person, only the first entry form received will be considered.

The decision of the VNA Foundation is final.

The VNA Foundation is not responsible for lost, misplaced or misdirected entries.

Information about the winner and finalists will be available on the VNA Foundation website (www.vnafoundation.net) after July 4, 2008.
Deadline
Completed nomination forms, and all accompanying information, must be received by the VNA Foundation at 20 N. Wacker Drive, Suite 3118, Chicago, IL 60606 no later than 5:00 p.m. on Thursday, May 8, 2008 to be eligible for the award. Forms and attachments may be emailed to vnafund@aol.com but must be received by the 5:00 p.m. deadline on May 8, 2008. No faxes will be accepted.
